Virgin Mobile has arranged for our customers to have access to this insurance program,
because without protection, you may be responsible for the full replacement cost of a
device in case of loss.
You are not required to purchase Phone Insurance in order to activate service with Virgin Mobile.
You may already have coverage through homeowner’s insurance or other means (your wireless
representative is not qualified to evaluate your existing insurance coverage). Keep in mind, if you
have other coverage, the deductible you would have to pay with another insurer may be higher.
If you choose to sign up for Phone Insurance, it may be cancelled at any time with a prorated

Phone Insurance is only available to Beyond Talk customers within 15 days of activation. To apply
for insurance for your new device, contact Virgin Mobile At Your Service at 1-888-322-1122 or by
dialing *VM from your Virgin Mobile device. Approval of your application is at the sole discretion
of the underwriter, Old Republic Insurance Company. If for any reason you are not eligible
for Phone Insurance, you will be notified within 30 days. In such an event, you do not have
coverage. If you have already paid premium, it will be refunded.

If you are approved for Phone Insurance, coverage begins on the day that your monthly
premium for Phone Insurance is due and paid.

Filing a claim is simple. Here’s how to do it:

Claim Replacement Conditions
· Claims may be fulfilled with new or remanufactured equipment. Remanufactured equipment may
  have minor cosmetic flaws.
· If the exact model is not available, replacement phone and covered accessories will be of like-kind and
  quality. Colors, features, and non-covered accessory compatibility are not guaranteed.
· An additional charge may apply if the deductible is collected via COD.
· Once your claim is approved and verified, your replacement equipment will be shipped to an
  address of your choice by overnight delivery (not including weekends).
· Damaged or recovered lost or stolen equipment must be returned within 15 days. A prepaid
  envelope with return shipping instructions can be found in your fulfillment packaging. Failure
  to return damaged equipment may result in the assessment of a nonreturn fee of up to $300.
· There is no cash payment or reimbursement provided to insureds. This program covers only
  the equipment that is assigned to the wireless phone number at the time of loss.
· Rates are subject to change upon reasonable notice to the insureds, in accordance with state law.
· All claims must be authorized by Asurion. The phone must be replaced by Asurion. Phone
  Insurance is a service provided to customers of Virgin Mobile USA. Asurion is the agent and

• Las reclamaciones pueden cubrirse con equipos nuevos o remanufacturados. El equipo remanufacturado puede
  tener defectos cosméticos menores.
• Si el modelo exacto no está disponible, el teléfono de reemplazo y los accesorios cubiertos serán de un tipo y
  calidad similares. No se garantizan los colores, las características ni la compatibilidad de los accesorios no cubiertos.
• Puede aplicarse un cargo adicional si el deducible se cobra contra reembolso (COD).
• Una vez que se apruebe y verifique su reclamación, se enviará su equipo de reemplazo a una dirección de su
  elección mediante servicio de entrega de un día para otro (sin incluir fines de semana).
• El equipo dañado o recuperado perdido o robado debe devolverse dentro de un plazo de 15 días. En su paquete
  de cumplimiento puede encontrar un sobre prepagado con las instrucciones de devolución. No devolver el
